Levels of Care:
We provide 4 levels of care appropriate to the needs of the hospice patient:
• Routine Home Care:
Regularly scheduled nursing care
provided in the patient's home.
• Continuous Care: Care provided
by hospice in the patient's home
when effective care is not
• Inpatient Care: 24-hour care
provided in an inpatient
contracted facility when pain and
symptoms cannot be managed at
• Respite Care: Temporary care
(up to 5 days) provided in an
inpatient contracted facility, in
order to provide rest time for the
